Patient Information – Travel Vaccination and Health Promotion Service

Deer Park Family Medical Practice strives to provide a Nurse lead travel and health promotion service from the Practice to all our patients travelling abroad for holiday and business.

The Practice asks that all patients complete the below Travel Form which is assessed by the Practice Nursing Team to determine if the patient requires any vaccinations, medication or health promotion depending on the patients’ destination (please state areas of a country you are visiting not just the country), holiday activities and the patients’ medical conditions.

This is a personalised service to specific patient need, to ensure all the information and guidance is as accurate as possible the Practice asks for a minimum of 6 weeks’ notice of the intended departure date for the form to be submitted.  If you are travelling within a 6 week period the Practice may not be able to offer you the above service and you will need to visit a Travel Clinic elsewhere.  It is important to note that some travel vaccinations take up to four weeks to take full effect.

The majority of the vaccinations are covered by the NHS and Public Health; however, some specific vaccinations and medications you will need to pay for, these come under the non-NHS services. 

Once the reception team have accepted the below FULL completed travel form, it will be date stamped and given to the Practice Nurse team for assessment.  The Nurse will look at the information given by the patient and advise if an appointment is necessary.  A doctor at the surgery will look over complex patients’ forms to ensure everything is correct to those specific patients’ medical needs.  The advice information will be added to the patients’ medical record along with a scanned image of the travel form.

Please call the surgery 10 working days after submitting the travel form, the reception team will then advise patients over the telephone if an appointment is required, and if there are any non-NHS charges to pay.  Patients will be asked to pay for any non-NHS services before the appointment with the Nurse as the Practice will need to obtain the appropriate vaccinations and/or medications.
