ORDERING MEDICATION
You can order medication on line, in person and by post. Please note that we do not take medication requests over the telephone. Once we have processed your request if you have nominated a pharmacy it will be sent electronically.
Please ensure that you allow at least 2 working days notice so that we can process your request. If sending by post please allow a minimum of 7 working days.

APPLICATION FOR ON-LINE SERVICES
To apply for our on-line services please collect the forms from reception at the practice, or download the forms by clicking on the link below. You will need to complete the form and bring it along with 2 forms of ID, one being photo ID (passport or Drivers licence) and the other a utility bill, bank statement or tenancy agreement dated within the last 3 months. The Clinical Lead at the practice will check the application. Once approved you will be contacted to come into the practice to collect your log in details in order to access the services.

To access our services you will need to register with the practice. Please note that you will need to live within our practice area in order to register.
A Registration Pack and GMS1 form can be collected from the reception desk at the practice; this will include a registration form, a short questionnaire and details of how to register for our on-line services. Alternatively you can download the pack by selecting the link below.

Did Not Attend - Missed Appointments
The Practice is experiencing a high level of 'Missed Appointments' by patients, We monitor the numbers monthly and this information is displayed at the practice. Patient who miss appointments on a regular basis will receive a letter from the Practice upon their second missed appointment, on the third missed appointment in a six month period the Practice will make an application for the patient to be removed from the practice list.
