Confidentiality
Information contained in patient health records are kept confidential at all times and only disclosed to others for purposes related to your health care (except if you have given permission otherwise).
Confidential patient data will be shared within the health care team at the practice and with other health care professionals i.e. hospitals referrals. All health care professionals have a contractual duty of confidentiality. Everyone working with the NHS has a legal duty to keep information about you confidential.

GP Net Earnings
All GP Practices are required to declare the average earnings for GPs working to deliver NHS services to patients at each practice.
The average pay for GPs working at Deer Park Family Medical Practice surgery in the last financial year, was £90,336.59, before Income Tax and National Insurance. This is based on 5 GPs who worked in the practice for 6 months or more.
It should be noted that the prescribed method for calculating earnings is potentially misleading because it takes no account of how much time doctors spend working in the practice, and should not be used to form any judgement about GP earnings, nor to make any comparison with any other practice.
Named GP
You may be aware that all General Practices are required to allocate a named GP to patients of all ages.
The named GP who will have overall responsibility for the patients care, planning and arranging support that may be required. Although your named GP will have overall responsibility for care planning for you this does not prevent you from seeing your chosen GP at the practice as you currently do.
If you do not know your allocated named GP please speak with a member of the Reception Team would will be more than happy to assist you.
Patients Charter
With your support we aim to offer you:
- An appointment within 48 hours if needed.
- Keep your wait at the surgery to a minimum.
- Offer emergency appointments – on the same day and are managed by a triage nurse who will direct you to the best source of help for your problem.
Please help us to help you by:
- Arriving on time for your appointment.
- Cancelling in good time if you do not require your appointment.
- Being courteous to all staff.
Please remember that you can cancel, change or alter an appointment 24 hours a day if you telephone us.
Privacy Notice for Digitalisation of Lloyd George Medical Records
Deer Park Family Medical Practice is currently in the process of digitising Medical Records. Scanning these paper-based records and making them digital will enable better utilisation of space, creating more clinical space, staff areas, multi team space potentially removing the need for some practices to build extensions. In addition, it will also make your record more easily and speedily accessible to clinical staff within your practice.
Your complete GP medical record will be digital and will be only accessible by your GP practice, with the paper-based records being securely destroyed. The scanning and destruction of the paper records will follow strict data protection guidelines adhered to by the NHS. As with paper based records, digital records are stored for the durations specified in the Records Management Codes of Practice for Health and Social Care
If you wish to discuss the scheme, please inform the Practice direct by letter to the Practice Manager at Deer Park Family Medical Practice.
Suggestions or Complaints
We hope that you will be satisfied with your care and treatment at the practice. We welcome any suggestions or comments you may have, likewise if you have a complaint please contact us. All complaints will be treated in total confidence.
Please ask at reception for a comments/suggestion form or alternatively contact the Practice Manager to discuss your concerns.
We aim to work together to resolve the issues.
Zero Tolerance
Please note that physical violence or verbal abuse will not be tolerated by this practice and any such behaviour by patients or their family could lead to them being removed from the practice list.
